Final Reports

• Final Report Form: Thank you for your feedback. With your input, we will be able to improve the quality of our Study Group program and develop new initiatives around teacher needs.
• Expense Report Form: Please itemize and include all original receipts attached to a separate 8 1/2 x 11" paper.

To obtain CPDUS:

The number of CPDUs you are eligible to receive is determined by the number of meetings you have (not by the number of hours).

• Team Members who have 3-5 meetings can receive 6 CPDUs.
• Team Members who have 6 or more meetings can receive 8 CPDUs. 

If you are interested in receiving CPDUs, you will need to provide the following four items when submitting your request to ISBE:

1.   A written statement of purpose for the group (e.g. a description of your Study Group’s Instructional Strategy—a copy of the catalog description may suffice)

2.   A list of your Study Group’s members

3.   Evidence of attendance (copy of sign-in sheets)

4.   Summaries of each meeting
