2009 Fund for Teachers Post Fellowship Reporting Guidelines

Fund for Teachers Fellows are required to provide the following post fellowship reports:

1. Expense Report

2. Two page post-Fellowship summary

3. Implementation Plan (submitted to the CFE Lesson Plan Database)

• THIS LINK will permit you to upload your unit and/or project lesson plans. If you have submitted a lesson plan in the past, simply use your email address and password to log in to the Lesson Plan Database.  If you have not submitted a lesson plan to our database before, please use your email address as your login, as well as the temporary password "cfeteachergrants"; you will be immediately prompted to choose a new password.  From there, please follow the directions to add your unit and/or lesson plan(s).

All reports should be sent together in one envelope. Teams should submit their reports together - please do not send them separately. Fellows are asked to submit their report forms to:
